HT Slipring Compartment Fire

No Fire Retarding Protection.

A fire that started in the HT Slipring Compartment in the revolving frame spread through the machine as there was no fire retarding protection on the machine.

This fire destroyed the machine.

It could have been prevented ...............

Hoist Drum Fault

No Hoist Slack Rope Protection.

During the lower motion of the bucket it struck the ground. The hoist drum kept on turning, causing the hoist ropes to pay out and coil themselves around the LV Collector. The operator not realizing what has happened reversed the hoist drum direction causing the hoist ropes to rewind destroying the LV Collector in the process.

It could have been prevented .................

Converter Cabinet Fault

No Spark or Fire Retarding Protection

This converter cabinet developed an oil leak. A spark from a loose wire caused the oil to ignite. The operator did not realize what had happened until the machine stopped. By that the fire was totally out of control.

This fire destroyed the machine.

It could have been prevented .................
